Best Bed Pillows for Back Sleepers: Medium Loft Picks

Lying on your back, your head needs lifting only a little. The right pillow fills the hollow of the neck without tipping the chin toward the chest, and it is still doing that at four in the morning.

The pillow for back sleepers

Back sleepers have the easiest job to describe and the one most often got wrong. On your back, the gap between head and mattress is small, so the pillow has to be lower than a side sleeper's. Too high and the chin tips toward the chest and the neck spends the night bent forward. Too low, or too soft, and the head drops back and the neck arches the other way. Neither feels like much at bedtime. Both feel like something by morning.

The numbers are not mysterious. The Sleep Foundation puts the right loft for most back sleepers at about 3 to 5 inches, and says most prefer a medium firm feel: enough resistance to hold the head up, enough give to let the back of the skull settle in while the fill rises to meet the curve of the neck. No one fill type wins. Down, feather, foam and latex all get there in different ways, and the choice between them is about how you like a pillow to feel and how much you want to fuss with it.

The five below cover those ways: a pillow-in-pillow down alternative, an adjustable shredded foam, a three chamber feather and down, a single block of soft foam, and a chambered American down pillow. Prices run from about $75 to about $179. Sleep on any new pillow for a couple of weeks before judging it, and if your neck hurts most mornings, see someone about it rather than buying a sixth pillow.

How we chose

  1. Medium loft. Roughly 3 to 5 inches under the weight of a head, with fill that stays there through the night.
  2. Medium support. Firm enough to hold the head, soft enough to let it settle without pushing the chin down.
  3. Fill that recovers. A feather core, foam or a chambered build so the pillow does not go flat under the head by morning.
  4. Care. A washable cover at least, and clear instructions for the fill.
  5. Sold properly. Every pick is sold on Amazon by Amazon.com or by the brand itself.

The 5 picks, in detail

Casper Sleep Original Pillow, Standard

Best overallNo. 1

Casper Sleep Original Pillow, Standard

Our take. A medium loft, medium soft down alternative built as a pillow inside a pillow, which is a sound design for a back sleeper.

Keeps for
A few years; synthetic fill flattens sooner than feathers or latex
Loft
Medium (maker's description)
Feel
Medium soft
Fill
Down alternative, pillow-in-pillow construction
Cover
100% cotton percale
Care
Fully machine washable
Seller
Amazon.com

Casper's Original is two pillows in one: a supportive inner pillow surrounded by a softer outer one, both filled with a down alternative fiber, inside a 100% cotton percale cover. The listing describes it as medium soft with a medium loft, and that pairing is close to what the Sleep Foundation says most back sleepers want. The inner pillow does the lifting; the outer one is what your head feels.

The whole pillow goes in the washing machine, which puts it ahead of every feather and foam pillow here on care. Casper says the fill resists clumping, which is the usual complaint about washable down alternative. The percale cover breathes well for a synthetic pillow.

Skip it if you like a pillow you can punch and mold, or if you expect it to last like down. Amazon sells it directly.

The catch. Down alternative compresses over time, and there is no way to add fill when it does.

Coop Home Goods Original Adjustable Pillow, Queen Size

If you want to set the height yourselfNo. 2

Coop Home Goods Original Adjustable Pillow, Queen Size

Our take. Shredded memory foam you take out until the pillow sits at the right height for your neck, which for a back sleeper means taking out a fair amount.

Keeps for
Several years; add fill as it compresses
Fill
Cross cut memory foam and microfiber, adjustable
Feel
Medium firm, set by how much fill you leave in
Cover

Coop Sleep Goods

Coop's Original is a zippered pillow filled with a medium firm blend of cross cut memory foam and microfiber, with a bag of extra fill in the box. It arrives full, at a loft meant for side sleepers. A back sleeper unzips it, removes a few handfuls, and tries it again, repeating until the head rests level and the chin does not tip forward. That is a slower first week than any other pillow here, and it is also the only way to get a loft set to your own neck rather than to a factory average.

The cover is Coop's Lulltra fabric, a polyester and bamboo derived viscose blend, and the pillow and outer cover are machine washable. Coop certifies the foam to CertiPUR-US and the pillow to GREENGUARD Gold, and backs it with a 100 night trial and a 5 year warranty. The shredded fill feels like a dense, slightly springy mass rather than a cloud, and it holds its shape all night.

Skip it if you want a pillow that is right out of the bag, or if you dislike the faint foam smell a new pillow gives off for a few days. Keep the fill you remove; the pillow compresses over the years and you will want to put some back.

The catch. It takes a week of unzipping and adjusting to get right, and the loose fill makes a mess if you rush it.

Pacific Coast Pillows Queen Size Set of 2, Goose Feather and Down 3-Chamber Hotel Pillow

The hotel pillow, two for the price of oneNo. 3

Pacific Coast Pillows Queen Size Set of 2, Goose Feather and Down 3-Chamber Hotel Pillow

Our take. A feather core inside two layers of down, the design most good hotel pillows use, sold as a queen pair by the maker.

Keeps for
Several years; feathers hold their structure longer than synthetic fill
Size
Queen, set of 2
Fill
Goose feather inner chamber, down outer chambers

Pacific Coast Feather

Pacific Coast calls itself the leading supplier of down pillows to American hotels, and this is its three chamber pillow for back and side sleepers. An inner chamber of goose feathers gives the support and keeps the pillow from flattening under a head; the chambers on either side hold softer down, so what you lie on is soft while what holds you up is firm. For a back sleeper it is a medium support pillow that still feels like down.

The fill is certified to the Responsible Down Standard and OEKO-TEX Standard 100, inside a tightly woven cotton shell that Pacific Coast says is down proof without a coating. The seams are double stitched with piping, and the pillow is machine washable. Two queen pillows for about $120 makes it the cheapest pillow here per head.

Skip it if you are allergic to feathers, or if you want a fixed shape. Feather and down pillows need a shake in the morning, since the fill drifts toward the edges overnight.

The catch. Feathers can work through the shell over time, and the loft cannot be changed.

TEMPUR-Pedic TEMPUR-Cloud Pillow, Standard, 2-Pack

The foam one, for a low and steady profileNo. 4

TEMPUR-Pedic TEMPUR-Cloud Pillow, Standard, 2-Pack

Our take. A single block of extra soft TEMPUR foam about 5 inches tall, made for back and stomach sleepers, sold by Amazon as a pair.

Keeps for
Years; TEMPUR material is slow to lose its shape
Size
Standard, 2-pack
Material
One piece TEMPUR foam, extra soft feel
Profile
About 5 in, low profile (maker's figure)
For
Back and stomach sleepers (maker's description)
Cover
100% polyester knit, removable, machine washable
Warranty
5 year limited

The TEMPUR-Cloud is one piece of Tempur-Pedic's slow responding foam in what the maker calls an extra soft feel, designed, in its words, for back and stomach sleepers. Tempur-Pedic lists it at about 24 by 15.75 by 5 inches and describes the profile as low. Under a head it compresses further, which lands a back sleeper in the medium loft range without any guessing.

Foam holds one shape all night, which is the appeal and the limit. There is nothing to fluff and nothing that drifts to the edges, and the material moulds to the back of the skull and the curve of the neck. The cover is a removable, washable polyester knit. Tempur-Pedic gives it a 5 year limited warranty and notes that, as a personal item, it cannot be returned once used.

Skip it if you sleep hot, since dense foam holds warmth, or if you like to fold a pillow under your head while reading. The 2 pack sold by Amazon brings the per pillow price to about $70.

The catch. Foam runs warm, and the slow response can feel like lying on something rather than in it.

Boll & Branch Down Chamber Pillow, Medium, Standard

If money is no objectNo. 5

Boll & Branch Down Chamber Pillow, Medium, Standard

Our take. American down around a feather and down core, in an organic cotton shell, filled and finished in the United States.

Made in
Filled and finished in the USA, imported shell
Keeps for
Many years, washed once a season
Size
Standard
Fill
Feather and down inner chamber, U.S. down outer layer
Support
Medium; also sold in Soft and Firm
Shell
100% certified organic cotton, down proof weave
Certifications
IDFL Down Standard, OEKO-TEX Standard 100, Fair Trade Certified

Boll & Branch builds its Down Chamber Pillow in three chambers. In the Medium and Firm versions, a feather and down inner pillow sits inside an outer layer of soft down from U.S. farms; the Soft version is all down. The Medium is the one for back sleepers, with enough feather in the core to hold the head up and enough down outside to feel like a proper down pillow. The Sleep Foundation names the Down Chamber its best hotel style pillow for back sleepers, and notes that the Medium and Firm suit most back sleepers better than the Soft.

The shell is tightly woven certified organic cotton, naturally down proof, from a family farm in India. The down is certified to the IDFL Down Standard, which traces it to the parent farm, and every component is tested to OEKO-TEX Standard 100. The pillow is filled and finished in the United States. Boll & Branch says to wash it alone in cold water once a season and tumble dry low, with wool dryer balls to restore the loft.

Skip it if $179 for one Standard pillow sounds like two pillows and a set of sheets, because it is. Boll & Branch sells it on Amazon itself.

The catch. The most expensive pillow here by a wide margin, for one pillow.

Compared.

Every comparison

Loft, support and what they mean lying down

Loft is how tall the pillow stands with nothing on it. Support is how tall it stays with your head on it. A soft 6 inch down pillow and a firm 4 inch foam one can put a back sleeper's head at the same height. What you want is the height under load: roughly 3 to 5 inches for most people on their backs, enough that the ear sits a little above the shoulder line and the chin points at the ceiling rather than the chest.

Your mattress moves the number. A soft mattress lets the shoulders and upper back sink, which closes the gap and calls for a slightly lower pillow. A firm mattress needs a slightly higher one.

The fills, briefly

Down is soft and moldable but compresses under a heavy head, which is why the good down pillows here hide feathers in a core. Feathers add structure and cost less. Down alternative is washable and suits allergies, and it goes flat soonest. Memory foam holds a shape and runs warm. Shredded foam can be adjusted and feels dense. None is best for back sleepers; the Sleep Foundation says as much. Choose the feel you like, then the firmness.

If you sleep on your back and your side

Most people do. A medium pillow with a feather core, such as the Pacific Coast or the Boll & Branch, copes with both because the feathers push back when you roll onto a shoulder. An adjustable pillow copes best of all, set a little higher than a pure back sleeper would choose. For a dedicated side sleeper pillow, see our guide to pillows for side sleepers; for the lowest, softest pillows, see the stomach sleeper guide.

If your neck hurts

A pillow that keeps the chin level is a reasonable thing to try first, and a cheap adjustable one is the least expensive way to experiment with height. But a pillow is not a treatment. If neck pain is persistent, comes with numbness or tingling, or follows an injury, see a doctor or physical therapist before spending more.

Frequently asked questions

What is the best pillow for back sleepers?

For most, a medium loft, medium support pillow such as the Casper Original, or an adjustable pillow like the Coop Original with some fill removed. If you want down, a chambered pillow with a feather core, from Pacific Coast or Boll & Branch, holds the head up where all down pillows sink.

How thick should a pillow be for a back sleeper?

About 3 to 5 inches under the head, according to the Sleep Foundation. Tempur-Pedic sizes its back and stomach sleeper Cloud pillow at about 5 inches unloaded. Broad shoulders or a firm mattress push you toward the higher end.

Should back sleepers use a firm or soft pillow?

Medium, leaning medium firm. Too soft and the head sinks and the neck arches back; too firm or too high and the chin is pushed toward the chest.

Is memory foam good for back sleepers?

Yes, if you like the feel. A low profile foam pillow like the TEMPUR-Cloud moulds to the neck and holds one shape all night. The trade is warmth and a slow, dense feel that not everyone enjoys.

Is a down pillow too soft for a back sleeper?

An all down pillow often is, since it flattens under the head. Look for a chambered pillow with a feather and down core, such as Boll & Branch's Medium or Pacific Coast's 3-chamber, which keeps the softness on the surface and the support underneath.

How often should I replace a pillow?

When it no longer springs back after being folded in half, or has gone lumpy or flat. Synthetic fill goes soonest; foam, latex and good feather pillows last longest.
