The pillows for neck support, by how you sleep

The job is geometry. Lying down, the head should sit so the neck continues the line of the spine rather than bending up, down or sideways. On your back that gap is small, a few inches, and the pillow should support the curve of the neck more than the back of the skull. On your side the gap is the width of a shoulder, so the pillow has to be taller and firm enough to stay that tall under the weight of a head. On your stomach the gap is nearly nothing, and Harvard Health's advice is blunt: the position is hard on the spine because the back arches and the neck turns, and the pillow should be very thin or absent.

Owners with neck pain report two things over and over. The first is that height matters more than material: a pillow that is too tall or too stiff keeps the neck flexed all night and they wake stiff, which is the same warning Harvard Health gives. The second is that a pillow which holds its shape until morning beats one that is perfect at bedtime and flat by three. Each of the five pillows here solves that second problem differently. One is a shaped block of slow foam. One is a zippered bag of shredded foam you fill to your own height. One is a slab of American Talalay latex that springs back. One is feather and down held tall by a gusset. One has a bladder of water underneath that levels itself as you turn.

None of this is medical advice. A pillow is a comfort object, and the right one is the one that lets you wake without a crick. If neck pain is persistent, wakes you, or comes with numbness or weakness in an arm, see a doctor rather than a pillow aisle.

How we chose

  1. Height you can set or trust. Either an adjustable fill, a choice of profiles, or a published loft that matches a sleep position.
  2. Holds its shape to morning. Foam, latex, a gusset or a water base: something that keeps the head where it was at bedtime.
  3. Washable and certified. A removable, machine washable cover at minimum, and CertiPUR-US, OEKO-TEX or an equivalent on the fill.
  4. Sold properly. Every pick is sold on Amazon by Amazon itself or by the maker, with a sleep trial or warranty where one exists.

Coop Sleep Goods

Coop's Original is a bag of cross cut memory foam blended with microfiber, medium firm, inside a zippered inner case and an outer cover of Coop's Lulltra fabric, a polyester and bamboo derived viscose knit. It ships full, at a height meant for side sleepers, with an extra half pound of fill in the box. The method is simple: sleep on it, and if the chin tips toward the chest, take a handful out; if the head tilts down toward the mattress, add some. Owners with neck pain report that this first week of fiddling is what finally gets them a pillow at the right height, because no factory average fits a particular pair of shoulders.

Shredded foam behaves differently from a solid block. It molds around the head and neck, it can be pushed up under the neck for a back sleeper, and it does not have the firm ridge of a contour pillow. It also stays where you put it until morning. The queen measures 19 by 29 inches. The foam is CertiPUR-US certified and the pillow GREENGUARD Gold, the cover and inner are machine washable, and Coop gives a 100 night guarantee and a 5 year warranty.

Skip it if you want a pillow that is right out of the box, or if you dislike the dense, slightly lumpy feel of shredded foam. The Talalay latex below gives a smoother, springier support at a fixed height.

The catch. A week of adding and removing fill before it is right, and shredded foam never feels like down.

Tempur-Pedic TEMPUR-Ergo Neck Pillow, Medium Profile, White

The contoured oneNo. 2

Tempur-Pedic TEMPUR-Ergo Neck Pillow, Medium Profile, White

This is the cervical contour pillow most others imitate. It is a single formed piece of Tempur-Pedic's slow responding TEMPUR material in what the maker calls an extra firm feel, with a raised lobe along one long edge that sits under the curve of the neck and a shallow dip behind it for the back of the head. Tempur-Pedic describes it as following the natural curve of the body to keep head, neck and shoulders aligned, for side and back sleepers. The cover is a removable, machine washable polyester knit, and the foam is CertiPUR-US certified.

Height is chosen before you buy, and that is the whole decision. The medium profile here is 20 by 12.5 by 4 inches at the neck lobe. Amazon also sells the small profile at 3.5 inches and the large at 4.75 inches, all sold by Amazon directly. A narrow frame or a back sleeper usually wants the small or medium; broad shoulders on their side want the large. Harvard Health's description of the ideal back sleeper pillow, a rounded support under the neck with a flatter area for the head, is more or less a description of this shape.

Skip it if you move between positions in the night. A contour pillow has a front and a back and only works one way up, and stomach sleepers should not use it at all. Owners with neck pain who stick with it report a week or two of adjustment before the firm ridge feels normal, and Tempur-Pedic notes that as a personal item it cannot be returned once used.

The catch. One shape, one height, one way up; choose the profile carefully, because it cannot be returned once slept on.

RejuveNite American Talalay Latex Bed Pillow, Queen High Profile

Talalay latex, made in ConnecticutNo. 3

RejuveNite American Talalay Latex Bed Pillow, Queen High Profile

Our take. A solid slab of American made Talalay latex, springy and open celled, from the company that makes the latex itself.

Rejuvenite

RejuveNite is the consumer brand of Talalay Global, the Shelton, Connecticut plant that produces Talalay latex for mattress and pillow makers, and this pillow is made there. Talalay is a particular way of making latex foam: the liquid latex is drawn into a sealed mold under vacuum, flash frozen at minus 22 degrees Fahrenheit to lock the cell structure before it is cured, then washed in five stages. The result is a more open, more even and more buoyant foam than the simpler Dunlop process gives, and it is why Talalay is the latex people mean when they say a latex pillow feels springy rather than dense.

For a neck, latex does two useful things. It pushes back, so the head does not sink through it by morning, and it recovers at once when you turn, so there is no slow dent to climb out of. It also breathes better than memory foam. This is the queen in the high profile, which the listing measures at about 7 inches unloaded; under a head it settles to a side sleeper's height. The pillow comes with a fitted liner and a 400 thread count cotton sateen cover that is machine washable, and the latex is certified to OEKO-TEX Standard 100 class 1, the strictest class, which covers products for infants.

Skip it if you sleep on your back or stomach; the high profile is too tall for either, and RejuveNite sells lower profiles. Skip it too if you want to fine tune. A solid latex pillow is one height, and this one is firm. Owners with neck pain who like it describe the feel as being held up rather than cradled.

The catch. Fixed height and firm; the high profile suits side sleepers only, and latex has a faint rubber smell for the first days.

Pacific Coast Goose Feather and Down Pillows, Queen Size Set of 2, Gusseted Pillows

Down and feather, held up by a gussetNo. 4

Pacific Coast Goose Feather and Down Pillows, Queen Size Set of 2, Gusseted Pillows

Our take. The traditional answer: two firm feather and down pillows with a 2 inch gusset so the edges stay as tall as the middle, from the maker that supplies American hotels.

Pacific Coast Feather

Pacific Coast describes itself as the leading supplier of down pillows to American hotels, and this pair is its firm, gusseted pillow for side and back sleepers. A gusset is the strip of fabric sewn between the top and bottom panels that gives a pillow straight sides like a box. Without it, a feather pillow domes in the middle and tapers to nothing at the edge, which is exactly where the neck rests. With a 2 inch gusset the height carries out to the edge, and the maker says plainly that the point is to fill the space between neck and mattress and keep head, neck and spine in line.

Feathers are the fill Harvard Health suggests trying first for a neck, because they conform to its shape, and they are the fill most people already know how to sleep on. The shell is 100% organic cotton with a double layer to keep quills in, and the down and feather are certified to the Responsible Down Standard. The listing notes that black cotton seed flecks visible in the cover are natural to unbleached organic cotton, which saves a confused return.

Skip it if you want the pillow to be the same height every night. Feathers settle, the pillow needs a shake each morning, and Harvard Health's caution that feather pillows collapse over time and should be replaced every year or so is the real cost of a $49 pillow. Skip it too if feathers make you sneeze; washing and sterilizing help but do not change the fill.

The catch. Loft drops over months and the pillow needs fluffing; feathers are the shortest lived fill here.

Mediflow Water Pillow with Memory Foam, Standard/Queen, Single Pack

The water pillowNo. 5

Mediflow Water Pillow with Memory Foam, Standard/Queen, Single Pack

Our take. A bladder of tap water under a layer of memory foam, set to the height you want by how much you pour in, and the only pillow design here with a clinical study behind it.

Keeps for
Years; the water base is refilled, and the maker sells replacement caps and funnels

Seller
Mediflow

Mediflow says it has made the water pillow for more than 30 years, and the idea has not changed. A sealed pouch in the base of the pillow is filled from the tap through a funnel; a layer of fill sits on top, here one piece of ventilated memory foam. You set low, medium or high by how much water you add, and the water does what no fiber or foam can: it flows to wherever your head is not, so the pillow stays at the same height when you roll from back to side and back again. That self leveling is why owners with neck pain who change position in the night end up here.

Mediflow cites a randomized crossover study, which it attributes to Johns Hopkins, published in the Archives of Physical Medicine and Rehabilitation in 1997. Forty one outpatients with benign neck pain slept a week on their usual pillow, then two weeks each on a roll pillow and a water based pillow. The water based pillow was associated with lower morning pain, more pain relief and better sleep quality; it did not change disability scores. It is one small study, but it is more evidence than any other pillow on this page has. Mediflow also registers the pillow as a medical device with the FDA. The maker's fiber filled version is the design in the study; this memory foam version adds a cradling top layer over the same base.

Skip it if weight matters. Filled, a water pillow weighs several pounds and does not travel. Skip it too if you want a soft, sinking pillow; the water base is firm and the foam on top is a thin layer, not a cloud. Fill it over the sink the first time, and expect a day or two to find the right amount.

The catch. Heavy once filled, firm underneath, and the water has to be topped up and the pillow kept away from anything sharp.

Compared.

Every comparison

Loft by sleep position

Loft is how tall a pillow stands empty; what matters is how tall it stays under a head. Side sleepers need the most, enough to fill the gap from ear to shoulder, which the Sleep Foundation's picks for the position put at roughly 4 to 5 inches under load and more for broad shoulders. Back sleepers need a medium height, about 3 to 5 inches, with support under the curve of the neck rather than under the skull. Stomach sleepers need the least, a soft pillow of 2 inches or so, or none, so the neck is not forced back as well as twisted.

Harvard Health's rule for side sleepers is worth remembering: the pillow should be higher under the neck than under the head. For back sleepers it suggests a rounded support for the neck with a flatter area for the head, which is what a contour pillow is and what a shredded pillow can be pushed into.

Firm is not the same as high

A firm low pillow and a soft high pillow can hold the head at the same height. The mistake owners with neck pain describe most often is buying a very firm, very tall pillow for support and waking with the neck bent toward the opposite shoulder all night. Choose the height for your position first, then choose a fill firm enough to keep that height until morning. If in doubt, buy lower and firmer rather than higher and softer.

Give it two weeks

A new pillow at a new height feels wrong for several nights even when it is right. Owners of contour and latex pillows in particular report a week or two before the neck stops noticing the change. Adjustable pillows should be changed a little at a time. If a pillow still hurts after two weeks of steady use, it is the wrong height, and if the pain is there whatever you sleep on, the pillow is not the problem.

Frequently asked questions

What is the best pillow for neck pain?

The one that keeps your neck in line with your spine for the way you sleep, and stays at that height until morning. For most people that is an adjustable pillow such as the Coop Original, because you can set the height to your own shoulders. Side sleepers who want a fixed pillow do well with a high profile latex pillow or a gusseted down one; back sleepers with a contour pillow like the Tempur-Pedic Neck Pillow. If pain is persistent or comes with numbness or weakness, see a doctor.

Are contour pillows good for neck pain?

Owners who sleep on their back, and some side sleepers, report they are, because the ridge supports the curve of the neck and the hollow keeps the head from being pushed forward. They do not suit stomach sleepers or people who change position often, and the profile has to be chosen correctly before you buy.

What pillow height do side sleepers need?

Enough to fill the gap between ear and shoulder with the neck straight, usually 4 to 6 inches under the weight of the head, and more for broad shoulders. Firmness matters as much as height, because a soft tall pillow compresses to a low one.

Is a latex pillow or memory foam better for the neck?

Latex pushes back and recovers instantly, so the head stays up and there is no dent to climb out of when you turn. Memory foam molds slowly and holds the shape, which contour pillows rely on. People who feel stuck in memory foam prefer latex; people who want the pillow to hold one exact shape prefer foam.

Do water pillows help neck pain?

A small randomized crossover study published in 1997, which Mediflow cites, found a water based pillow associated with less morning neck pain and better sleep than the subjects' usual pillows or a roll pillow. The practical benefit is that the water levels itself as you change position, so the height does not change in the night.

How often should a pillow be replaced?

When it no longer holds its height. Harvard Health suggests feather pillows be replaced every year or so because they collapse. Shredded foam can be topped up; solid latex and TEMPUR foam keep their shape for years.
